- 1). Open the web browser on your computer and go to the DSHS Vital Records Application site (see Resources).
- 2). Select "Birth Certificate" from the drop-down menu and click "Continue."
- 3). Select the state that issued your driver's license or identification card from the drop-down menu. Select your relationship to the person whose birth certificate you are requesting from the drop-down menu and enter the date of birth on the birth certificate you are requesting.
- 4). Check the box indicating you agree to the terms and conditions of the website and click the "Continue" button.
- 5). Enter the requested information about the birth certificate you want to request, and your personal information including your current address and driver's license or identification information. Click the "Continue" button.
- 6). Enter your credit card information to pay for your order. Verify and confirm your information and click "Continue" to place your order.
