- 1). Report your passport as stolen or lost. You must complete form DS-64 in order to report your passport lost or stolen. The passport will become invalid and will no longer be able to be used for travel. That means that if the passport is recovered after it has been reported it will still need to be replaced. Keep in mind that not all post offices process passports so it is best for you to check the website and find the closest facility to you so you don't waste time driving around. Call before you go because some only accept people by appointment.
- 4). Complete form DS-11 to apply for a new passport and if your lost passport was still valid you should submit form DS-64 with your DS-11 at your local passport agency or acceptance facility. Make sure you complete these forms accurately and write legibly in blue or black ink. Not writing legibly will result in a rejected application or an increased possibility of errors on your passport.
