- 1). Obtain and complete the application for a passport. You can find the appropriate application online (see Resources). Download and complete the application. There are different applications for the following categories: first time applicant (adults 16 and older); minor applicant (age 15 and under); and passport renewal.
If you cannot access the forms online, visit any of the passport offices in Cleveland listed below. - 2). Obtain two passport-sized photos. The photos must be 2 inches by 2 inches in size, and your head and shoulders must be centered and clear. You can have the photos professionally taken at most drug stores or photo-finishers. Some of the passport offices also take the photos for you, through a drug store can be less expensive.
- 3). Bring the completed passport application and photos to any of the passport offices in Cleveland. You also will need to bring proof of U.S. citizenship. To prove citizenship, bring an original or certified birth certificate, a naturalization certificate, or an expired passport.
