- 1). Verify that you are eligible to apply for a name change. Anyone who wishes to change their name in British Columbia must be at least 19 years old and have lived in the province for at least three consecutive months before making an application. For an applicant under 19, a parent with custody of the child can apply for a name change.
- 2). Obtain a change of name application package from the British Columbia Vital Statistics Agency office or website, or from a Service BC office. If you are changing your maiden name after marriage or reverting to your maiden name, contact the Vital Statistics Agency as you may not have to apply for a name change.
- 3). Ensure you have the necessary documentation to support your application. All original Canadian birth certificates, certified copies of immigration papers or the applicant's permanent resident card must be submitted. A marriage certificate and proof of custody when a parent is applying for a child's change in name may also be needed.
- 4). Obtain a fingerprint package from the Vital Statistics Agency. It is required for all applicants 18 years old and older. Fingerprints are taken at a local police office, a Royal Canadian Mounted Police detachment or another approved agency.
- 5). Submit your application form with the applicable fee at a local police office, RCMP detachment or other approved agency. After taking the applicant's fingerprints, the agency will send the prints and the application form with any supporting documents to the Vital Statistics Agency. Fingerprints will be sent to the RCMP for a criminal record check.
- 6). Request a change of name certificate after your application is approved. Certificates can be requested by anyone with written authorization of the applicant, a legal guardian, or an executor of the applicant's estate.
- 7). Apply for new government identification cards. This includes requesting a new birth certificate, citizenship or permanent resident card, health card and drivers license. You will also have to inform your bank, employer, utilities and other applicable agencies and businesses about your change in name.
