There are several different ways to view your Michigan driving record. You can use the most convenient method that works with your schedule and needs.

First you will need to get Michigan State Department form BDVR-153, available at any branch office or online at:

Once you complete form BDVR-153, Mail the completed form and payment to:

Michigan Department of State
Record Lookup Unit
7064 Crowner Drive
Lansing, MI 48918-1502

The lookup fee is $7 and the official driving record fee is $1. You can pay by Discover card, Mastercard, or VISA by completing your card information on the form. You can also pay with a check. The record will be mailed to your mailing address on record with the State Department. If you need to have the record mailed to someone else like an employer then check the box in Section 1 and fill out Section 5. You should expect to receive your driving record by mail in 3-8 weeks.

You can also send the form by Fax. Fax the completed form to (517) 322-1181. You will have to pay by Discover card, mastercard, or VISA using this method. The driving record will be mailed to your mailing address on record with the State Department. If you need to have the Michigan driving record mailed to someone else like an employer then check the box in Section 1 and fill out Section 5. You should expect to receive your driving record by mail in 3-8 weeks.

If it is easier for you, order the Michigan driving record by telephone at (517) 322-1624. Using this method you will have to pay by credit card (Discover card, mastercard, or VISA). The Michigan driving record will be mailed to your address on record with the State Department. If you need to have the Michigan driving record mailed to someone else like an employer then check the box in Section 1 and fill out Section 5. You should expect to receive your driving record by mail in 3-8 weeks using this method.

If you are in a hurry and need the driving record right away, your best option is to visit any State department branch. You will not need to fill out the form, but you will be required to present your Michigan driver's license. The fee is $8 and is only payable by cash or check.
