- 1). Visit the Illinois Vital Records website and click on the link marked "Birth Records." Choose the "How to Order Online" link and read the shipping and fee rules. When you're finished, click the link at the bottom of the page to order your birth certificate.
- 2). Check the "Birth Certificate" box and click "Continue." On the next page, choose whether or not you're ordering the birth certificate for yourself or for another person; if you're ordering a birth certificate for someone else, enter that person's legal name. Click "Submit."
- 3). Enter the name of the city where the birth certificate holder was born and click "Continue." Enter the birthdate of the birth certificate holder and click "Continue." Select the reason you will be using the birth certificate.
- 4). Choose whether or not you want your birth certificate delivered by regular mail or expedited. Click "Select." Read the information and click "Submit." Choose whether or not you'd like additional copies and click "Continue." Choose your gender and click "Continue."
- 5). Enter the names of the parents. If both are listed on the birth certificate, click "Continue."
- 6). Enter your address and personal information on the next page. Click "Continue" and enter your American Express or Visa credit card information. Click "Continue," review your order and click "Submit Order."
- 1). Visit the Illinois Department of Health Division of Vital Records Office at 925 East Ridgely Ave. in Springfield, Illinois. The office is open Monday through Friday from 10 a.m. until 3 p.m.
- 2). Request a certified copy of the birth certificate you need from a Vital Records employee. Present your photo identification and place your order.
- 3). Pay the fee associated with obtaining a certified copy of your birth certificate. A Vital Records employee will tell you the final cost based on the number of copies and choice of delivery. Vital Records accepts credit cards, checks and money orders.
