How to Obtain a Birth Certificate From Mayaguez, Puerto Rico

    • 1). Obtain a birth certificate application by visiting the Demographic Registrar's Office page on the government website of Puerto Rico or call 787-767-9120 to request an application by mail.

    • 2). Complete the application with information as listed on the birth record including the reason for request and the applicant's relationship to the subject on record.

    • 3). Provide mailing address to which the certificate will be sent. Sign and date the application.

    • 4). Make a photocopy of the driver's license or passport of the person sending the request. A copy of the marriage certificate is also required if the applicant's last name differs from the birth record due to marriage.

    • 5). Mail the completed application, photocopies and self-addressed, stamped envelope to the Demographic Registry.

    • 6). Include a money order payable to the Secretary of Treasury of Puerto Rico. As of December 2010, the cost is $5 for one copy and $4 for each additional copy.

    • 1). Register online at the Government of Puerto Rico website to submit the application electronically.

    • 2). Provide a valid email address and digital copies of photo-required documents.

    • 3). Furnish credit card information for payment of the certificate, as well as an additional $1.50 for shipping.

    • 4). Log in to your account to check the status of your application.
