    • 1). Research where you need to apply for a change of name. This is different in every state. Visit your local probate court, call your local town hall or visit your local law library to ask the clerk.

    • 2). Obtain all forms necessary for a change-of-name petition in your state. This may include the petition, a legal backer form and a notice to the public of the name change.

    • 3). Fill out and sign the forms with a pen. Have the forms notarized, or sign them in front of a county clerk. Apply the paperwork with your local government with any applicable fee in the form of a United States Postal Money Order.

    • 4). Await a response from your local government. A form declaring your name change arrives in the mail. Use the proof of your new name to replace any licenses, credit cards or passports written in your old name.
