- 1). Confirm that you are eligible to apply for a birth certificate. In order to be eligible, you must be at least 18 years of age and either be the person named on the birth certificate or an immediate family member. Also, individuals with an active Power of Attorney may apply for a birth certificate on behalf of another individual.
- 2). Print out the application for a Certified Copy of Birth Record from the Pennsylvania Department of Health (see Resources). Complete the application. You will need the name at the time of birth, parents' names, place and date of birth, gender, and applicant's contact information. Sign the application after all information is complete.
- 3). Submit the completed application along with $10 to the Department of Vital Records. Only send checks or money orders. Any application received with cash will be returned to the sender. Also, attach a photocopy of your valid photo ID. Mail to:
