- 1). Decide how you would prefer to apply. Applying in person may speed up your application, but applying through the post is usually more convenient.
- 2). Check that you meet the requirements. To change your birth certificate in New York, you need to be 18 years of age or older, and either be the person named on the certificate or a parent/guardian of that person.
- 3). Download the application form. (See Resources)
- 4). Fill this in as accurately as possible, and fill in the payment details. Most changes cost $40, with an extra $15 charge for copies of the new certificate.
- 5). Check which documentation you will need. (See Resources) Enclose a good copy of each item, along with the original, to speed up the processing time.
- 6). Include a self-addressed, prepaid envelope if you are applying by post, to ensure your documents are returned.
