How to Replace a Lost Birth Certificate in Arizona

    • 1). Obtain and fill out the Application for a Certified Copy of a Birth Certificate provided by the Arizona Office of Vital Records. This form is available online (see Resources) or in person at several local county offices (see the online form for a list of participating offices).

    • 2). Copy your government-issued ID, front and back, and include this with your application. If your ID does not have a signature, you will need to sign the copy in the presence of a notary and have the copy notarized. If you are an immediate relative of the named person on the birth certificate, you will also need to copy and include proof of your relationship to the named person.

    • 3). Mail the application and supporting evidence along with the $10 fee to a participating county office listed on the application. The applications and payment may also be made in person. It can take up to 21 days to receive a birth certificate depending upon the office and request.
