Redefining Personalized Medicine in the Primary Care Setting
Personalized Medicine 3.0
At its best, personalized medicine will encompass an integrated evidence-based approach to individualized patient care across the continuum from health to disease. Ideally, practitioners will use a multidisciplinary structure to promote health and wellness, patient education, and satisfaction and customize disease prevention, detection, and treatment on an ongoing basis (Figure).

Here is a description of the future: Personalized Medicine 3.0.
Before her appointment, Samantha has genetic testing completed at home using a buccal swab. Data from that sample are analyzed, and the results of Samantha's genetic testing are automatically uploaded into her EHR.
Samantha wears a painless skin sensor that monitors her blood glucose levels and activity levels over a 1-month period. Monthly summaries of these data are uploaded into her EHR and crosschecked with her biometric data, family/social history, medication and allergy lists, and data from risk calculators.
Recommendations to alter Samantha's risk of developing these conditions are suggested to the provider, along with pharmacologic recommendations based on her pharmocogenomics profile.
Community resources to help and support Samantha to engage in healthy behaviors that are available in the zip code where Samantha lives and works are identified and presented to Samantha and her provider for consideration.
