- 1). Fill out the Arkansas Birth Certificate Application (see resources). You will need information such as the applicant's full name at birth, date of birth, place of birth, current age and parents' names.
- 2). Secure a $12.00 check or money order payable to the Arkansas Department of Health. You can request multiple copies of a birth certificate for an extra $10.00 each.
- 3). Put the application and money order in an envelope and seal it.
- 4). Address the envelope to:

Little Rock, AR 72205 - 5). Place the first class stamp in the upper right corner of the envelope. Mail it.
