How to Replace a Lost Birth Certificate in Ontario
- 1). Find a friend, family member or co-worker who can act as a guarantor, confirming with the Office of the Registrar General that you will provide accurate information when requesting to replace your lost birth certificate.
- 2). Visit the Office of the Registrar General's website at https://www.orgforms.gov.on.ca to access the official form to request a replacement birth certificate. Fill out the form, giving them your current address, the date of your birth, your parents' names, and the name and contact information of your guarantor.
- 3). Provide your credit card information for payment. Your replacement birth certificate costs $35.
- 4). The birth certificate will arrive in the mail within 15 days. If you have not received the birth certificate by then, check the Office of the Registrar General's website for the status of your certificate, or contact the Office to receive a refund.
