How to Get on Original Copy of an Oklahoma Birth Certificate

In-Person Request

• 1). Open the Oklahoma State Department of Health web page and click "Birth and death certificates."
  
  
• 2). Click "Birth certificates."
  
  
• 3). Scroll to and click "Application for search and certified copy of birth certificate."
  
  
• 4). Complete the birth certificate application. You can complete the application online and print it or you can print the application and manually complete it.
  
  
• 5). Visit the vital records or health department and submit the application with your proof of identification and your payment for the total number of certificate copies you wish to obtain. Applications submitted in person are provided the same day.
