How to Complete Chapter 7 Colorado Reaffirmation Agreements
- 1). Obtain a reaffirmation agreement form from the U.S. Courts website.
- 2). Fill in "U.S. Bankruptcy Court for the District of Colorado," your name, case number and "Chapter 7" at the top of the form. Fill in the name of the creditor, and check the box if the creditor is a credit union.
- 3). Describe the original agreement that is being reaffirmed.
- 4). Fill in the amount of the debt being reaffirmed and the date the disclosure was prepared.
- 5). Fill in the annual percentage rate.
- 6). Indicate the repayment terms of the agreement.
- 7). Complete the next two steps, if you are not represented by an attorney or if your creditor is not a credit union. Otherwise, skip to Step 10.
- 8). Fill in information about your current income and expenses.
- 9). Check whether reaffirming the debt will place an undue hardship on you.

Indicate whether the debtor believes that reaffirming the debt would be in his best interest and check "No Presumption of Undue Hardship," if you are the debtor's attorney, and the creditor is a credit union. - 11
Sign and date the certification. Include the creditor's name and address, and print the name of the creditor's representative who negotiated the reaffirmation agreement. Have the representative sign and date the reaffirmation agreement. - 12
Get your attorney, if you have one, to fill out the certification that you were represented by him in negotiating the reaffirmation agreement.
