How to Replace a Lost Birth Certificate in New Brunswick

    • 1). Print a copy of the New Brunswick Application for Birth Certificate or pick up a copy at your local Service New Brunswick office. If you're applying online through the SNB Online Application for a New Brunswick Birth Certificate, go to the application web page by clicking the link in the Resources section, read the information on the first page, click the "Continue" button, and then follow the instructions on each page that follows.

    • 2). Fill out the "Part 1: Applicant Information" and "Part 2: Birth Details" sections of the application. Provide your name, contact information and relationship to the person named on the certificate. Under "Part 2: Birth Details," provide the person's vital birth information, including the person's name at birth, given name(s), date of birth and place of birth; mother's maiden name, given name(s) and place of birth; and father's name, given name(s) and place of birth (if applicable/available).

    • 3). Complete the "Part 3: Certificate Details" section. Select the type of birth certificate you want (long or short form) and whether you want it in English or French. Select the reason for making your request by checking off "Lost/Stolen" and circling "Lost." Choose a birth certificate delivery option and payment method (Visa, MasterCard, American Express, check or money order). If using a check or money order, make it payable to "SNB." Make sure to include both the cost of the birth certificate and the delivery charge.

    • 4). Take the application to the person named on the certificate and have that person fill out the "Part 4: Consent" section. If you're requesting a copy of your birth certificate, or the certificate of your child (18 or younger) or a deceased person, skip this step.

    • 5). Sign and date the application and submit it to Service New Brunswick, along with your payment and any supporting documentation, such as a copy of your identification, proof of guardianship or legal responsibility, court order or proof of death. Go to the SNB "Our Current Processing Times for Birth, Death & Marriage Certificates" web page (see Resources) for updated processing times, and then submit your application in person at a local SNB office or via fax or mail to the Vital Statistics Office:
