How Can I Correct a Mistake on an Original Birth Certificate?
- 1). Gather information that supports the reason for the change. Include doctor information, hospital records, DNA paternity proof or any other notarized documentation. Show the vital records office original copies of the paperwork. Scans and photocopies are not accepted.
- 2). Request an application for amendment to birth certificate from the state department of health or other state department that deals with vital statistics. The names of the forms vary slightly different by state.
