- 1). Gather information that supports the reason for the change. Include doctor information, hospital records, DNA paternity proof or any other notarized documentation. Show the vital records office original copies of the paperwork. Scans and photocopies are not accepted.
- 2). Request an application for amendment to birth certificate from the state department of health or other state department that deals with vital statistics. The names of the forms vary slightly different by state.
- 3). Fill out the application with the name of the person who is requesting the new birth certificate, the date of birth, place of birth, mother's maiden name, parents' names, the type of change you seek, your address and the reason for the change.
- 4). Return the application along with the processing fee, which will vary by state. Pay additional fees for a rush request if you need the birth certificate changed quickly.
