The Arizona Department of Health Services handles death certificates for the state. You can request a death certificate if the death occurred in Arizona. The fee for a certified death certificate ranges between $10 and $15, depending upon the county where the death certificate is located. - Only people age 18 and over are eligible to request a death certificate.
- Arizona's death certificates are not a matter of public record. The state does not allow public access to death certificates.
- If you request a death certificate by mail, you must sign the death certificate application in front of a notary before mailing it, or provide a copy of your valid driver's license or state-issued identification card. The same rules apply when requesting a death certificate in person.
- Arizona law permits only certain categories of people to obtain death certificates. The spouse or immediate family of the deceased are eligible, as well as attorneys who can prove a legal interest in the case, or who are authorized by immediate family members to get a copy. Private investigators and other third parties are eligible if they are authorized by immediate family members of the deceased and can also prove their legal interest in obtaining a death certificate.
- Only parents of a deceased fetus can request a copy of a death certificate.
