- 1). Access a form site that creates forms for free. Click the "Sign up for Free" button at the top right corner. Enter your information and click "Ok." You are automatically sent to a webpage where you can create a form.
- 2). Click the "Create a Form" link that is displayed on the first page. This brings you to a list of options where you choose the type of free form to create. Choose a template like "Website Evaluation" or "Registration." You can also choose a custom template by selecting "From a blank form." Click the "Create" button when finished.
- 3). Choose the labels and form values. These are the entries that will be obtained by your readers. For instance, typical form values are "First Name," "Last Name" and "Address."
- 4). Click the "Preview" button to view your form before publishing. Ensure the labels are properly set and the layout is easily navigated by the user. Close the preview window once you've confirmed the form layout.
- 5). Click the "Publish" button to save the changes and make your form available. The next screen gives you links that you can use to publish your new form. Use the "Embedded Link" code to insert the form into your existing webpage or use "Form Link" button to send users to the free form website.
