- 1). Print out the birth certificate application from the Utah Department of Health website (see Resources) and fill out all of the requested information.
- 2). Make an enlarged copy of your driver's license, military identification or state-issued identification, or two copies of your Social Security card, marriage license or W-2 form. A complete list of identification that can be used is attached to the application.
- 3). Make a check or money order payable to the Utah Department of Health in the amount of $18 for the first copy. Add $8 for any additional copies.
- 4). Mail the completed form, identification and payment to:
