- 1). Download an application from the Healthy Families website or request an application by calling (800) 880-5305 Monday through Friday between 8am and 8pm. Fill out the application completely.
- 2). Obtain copies of papers showing proof of income including a copy of your 1040 from the previous year. Send copies of proof of citizenship and proof of residency in California. Acceptable forms of proof include a birth certificate or a utility bill.
- 3). Seal the application and copies of the necessary paperwork in the envelope included with the application.
- 4). Address the envelope to: Healthy Families/Medi-Cal, P.O. Box 138005, Sacramento, CA 95813-9984.
