How to Correct Mistakes on a Foreign Birth Certificate

Call the embassy or consulate of the country of birth or visit the country's website (if available) to procure the name and contact information for the department or division of vital statistics and the associated local office in the geographic location of birth that handles vital birth information.

    • 2). Contact the office of vital statistics or visit the department or division webpage to obtain instructions on submitting a request to amend or correct a birth certificate. Ask a representative to mail you an application, or download the form from online and then print a hard copy.

    • 3). Fill out the application legibly following the instructions outlined for the country of birth. Typically, this involves entering the original birth certificate information such as the baby's name, parents' names, mother's maiden name, date of birth, place of birth, and birth status (biological child or adopted) into the fields provided; as well as noting the errors and corrections.

    • 4). Submit the application via fax or mail. Make certain that you include one or more pieces of proof of your identification such as a copy of a driver's license, passport, social security or other government ID card, signed doctor's statement, or school records, as well as any requested supporting documentation (copy of original birth certificate, marriage certificate, family registry, or court decree). In addition, supply a written statement or affidavit explaining the circumstances under which the error occurred and pay any applicable fees.
