How to Obtain a Birth Certificate in Boulder County

• 1). Go to the Boulder County Public Health Office, Vital Records department in Boulder, Colorado.

  Pay with cash, money order, credit card or personal check. As of April 2010, the cost is $17.75 for the first copy. Any additional copies of the same document are $10 a piece. You will receive the birth certificate within 10 to 15 minutes.

• 1). Order a copy through the mail by sending a photocopy of the appropriate identification and a check payable to "Boulder County Public Health Vital Records." The cost is the same by mail as it is in person.
  
  
• 2). Include a Birth Record Request. Download the form from the Boulder County Public Health website and fill it out completely. The form is available in English and Spanish.

  Request a birth certificate online, through the VitalChek website. You will have to pay with a credit card when ordering online. You can find the link for VitalChek on the Boulder County Public Health website.
  
  
• 2). Choose "Birth Certificate" on the VitalChek homepage. Follow the step-by-step instructions for applying for the copy.
  
  
• 3). Complete the application process and provide all the requested information. Have the city, state and county of birth for the person named on the birth certificate handy. You will also need to enter his or her name as it appears on the birth certificate, the date of birth and the names of both parents listed on the certificate (including the mother's maiden name).
