- 1). Download and print the N-565 form from the U.S. Citizenship and Immigration Services (USCIS) webpage. The N-565 form is the form that you fill out when you need a copy or replacement citizenship certificate. You can find the application form in the Resources section.
- 2). Read the instructions carefully and fill out the N-565 form. You will need to use a black pen to fill out the form. Also, if you are requesting a form because you have damaged the original certificate, you must include the damaged original certificate with the application form.
- 3). Include a check or money order for $380 as of 2010 made out to the U.S. Department of Homeland Security.
- 4). Mail the form to either the Texas Service Center or the Nebraska Service Center. The center where you mail the form will depend on the state or country in which you reside. Make sure that you read the application form careful and send the completed form to the correct service center.
- 5). Wait for the application to be processed. If the USCIS requires additional information from you, it will contact you. Otherwise, it could take several weeks to process your request. If you have any questions about the status of your application, call the USCIS at 800-375-5283 (800-767-1833 for the hearing impaired). If you are currently outside of the United States, contact the United States Embassy in the country you are in and speak to an official there about getting a copy of a citizenship certificate.
