    • 1). Assemble your supporting documents: birth certificate or other proof of citizenship, photo identification and marriage certificate.

    • 2). Obtain an application for name change, Form SS-5, from your local Social Security Administration office or from the first reference on this page. Your local office can be found by following the second resource on this page, or by looking through your local phone directory.

    • 3). Complete the application and submit it to the SSA office. Take the form and documents directly to the Social Security office or mail them to your local branch. Your official documents must accompany the form, so mailing is not a desirable option for most people, but it is allowed.

    • 4). Notify your employer of your name change so that your employer and the SSA have the same information. Your new Social Security card will arrive as soon as your application has been processed. Hand-delivering your documents and form will expedite the process, but you can expect to wait two weeks to a month for your new card.
