- 1). Search the Internet for a DOT-certified physician. Make a list of physicians in your area.
- 2). Call the doctor's office to schedule the DOT physical exam. Go to the doctor's office at the appointed time.
- 3). Submit to the physical exam. The doctor will fill out and give you the DOT health form after successful completion of the exam. This DOT form is the health "card" that is needed to obtain a commercial driver's license.
