Conclusion
There is clearly a need for improved diagnostic and prognostic tests for insect sting allergy. Currently available tests have up to 95% accuracy, but when the diagnosis is difficult, there is an additional role for component-resolved diagnosis using recombinant allergens and for BATs. There is a need to standardize the materials and methods for these tests to avoid some of the discrepancies that are evident in the literature. Another diagnostic tool that would be most valuable is the ability to identify the patients who are at highest risk for the most severe reactions to stings. In this respect, the BAT holds some promise, but ideally it would be assessed in studies using live sting challenge for the most controlled observations of its correlation with clinical outcomes. Measurement of baseline serum tryptase or other mast cell mediators has shown significant correlation with severity of sting reactions.
