consider. Some of the thoughts that may come to mind are:

There are few constraints regarding age for breast reduction surgery. Generally, the surgeon will want to wait until the breasts are fully developed. Another consideration

is the changes that take place during breast-feeding. Childbirth and breast-feeding can cause many different changes in the breasts. Often women will choose to have

breast reduction before bearing children. They consider the consequences and choose to face the subsequent changes later in their lives. This topic needs to be

discussed with your surgeon before surgery is performed.

The plastic surgeon will discuss your desires about the breast reduction. What do you feel will be the best size for your body frame and comfort? Are you expectations

realistic?

During your initial consultation for breast reduction surgery the plastic surgeon will examine your breasts closely. He/she will take measurements and possibly

photographs for your records. Your skin quality and the placement of your nipples are areolas will be evaluated as well.

The plastic surgeon needs to have a complete and accurate medical history. Information such as drug allergies, any chronic conditions, previous medical treatment or

biopsies as well as current medication that includes both prescription and over-the-counter medications you are taking.

If you are overweight, and planning on significant weight reduction, this needs to be discussed with the surgeon. Chances are you will be advised to get your weight

stabilized before undergoing breast reduction. It is generally known that weight loss will cause a reduction in breast size.

Some individuals who have lost a large amount of weight choose to have other procedures done at the same time as their breast reduction. They may also decide to

have a tummy tuck, or liposuction procedure included in their post-bariatric surgery.

Insurance Coverage

Some conditions will prompt insurance companies to pay for your breast reduction. Some of the considerations are the amount of tissue that will be removed and the

terms of your policy. The physician may need to submit a letter of recommendation to the company stating all of the factors involved in the need for a breast reduction.

It is generally someone in the office that will discuss this with you prior to surgery.

Types of Breast Reduction Surgery

The most common procedure will remove excess tissue, skin and fat through an incision. The nipples and areola remain intact to the tissue but will be shifted to a

higher position. The areola may be reduced in size at this point. The skin that was above the nipples will be brought down and joined to reshape the breast.

Generally, there are three incisions made during the procedure. One incision is made around the areolas. the second will run vertically from the crease under the breast

to the bottom of the areolas. the third incision will follow the curve of the breast crease.

Often when breasts are very large, the areola are also stretched. They may be reduced in size to keep proportions in place. Liposuction may also be used to remove

excess fat.

Generally the areolas and nipples will remain attached to the underlying tissue. This will preserve sensation and in some cases, but not all, will allow the patient to

breast-feed. This is an inverted T-shaped procedure. Although the inverted-T is presumably the best for large breasts, it is also the one that causes more scarring.

Usually the scars are in a position that can be hidden with clothing.

Procedure Variations

Every surgeon has a technique for performing breast reduction. Some of the factors that are considered are the size and shape of the breasts and the amount of

reduction that is desired. These factors will enable the plastic surgeon to plan the type of surgery that is the best technique for each patient.

The short scar or vertical incision usually will result in less scarring. The vertical incision breast reduction are popular for women who do not require an excessive

amount of tissue removal.

The incision for this procedure will extend around the areola and down to the breast crease. There is no need for a horizontal incision and consequently less scarring in

the crease. This procedure is best suited to the woman that needs a moderate amount of tissue removed. It is not commonly used for women who have to have a large

amount of tissue removed.

A third type of breast reduction is liposuction. Liposuction will have a minimal amount of scarring. It is also important to realize that liposuction will only remove fatty

tissue. For this reason, the procedure is recommended for women who have an excess of fatty tissue and minimal glandular tissue. Liposuction will remove the weight

of large breasts but will not give the tightened or lift results of the other procedures.

Risks of Breast Reduction Surgery

Although complications from this type of surgery are uncommon, it still needs to be discussed with the surgeon. Thousands of these procedures are done annually and

most women have no major problems and find the results to be acceptable. It is still important to discuss potential risks.

There are potential complications. One such complication is anesthesia. This is a risk in any type of surgery. Some of the other potential complications are excessive

bleeding and infection. It is rare that a transfusion is required during the procedure. Some surgeons will request their patients to donate their own blood before surgery

in preparation for the potential need.

After reduction, the breasts may not be perfectly symmetrical. The nipple height may also vary. There can be minor adjustments made to correct these difficulties later.

On occasion, there can be permanent loss of sensation in the breasts or nipples. This is rare, but possible. There will be times when a revisionary surgery can be

performed if the incision has not healed properly or in the event of injury or loss of the reconstruction skin graft.

Most risks can be avoided if the patient follows the surgeon's advice both before and after the procedure is performed.

What Will Be Done Before Surgery

Several steps need to be taken prior to surgery.

Depending on your age and breast cancer history in your family, a mammogram may be ordered and repeated after surgery. The mammogram after surgery will be done

several months after breast reduction to determine any significant changes in breast tissue. You will still be encouraged to perform breast self-examination. This type of

surgery does not increase the risk of breast cancer.
