- 1). Visit the Social Security Administration's Disability Benefits online application tool (see References).
- 2). Click on "Adult Disability Checklist" under "Follow these 4 easy steps to apply online" near the bottom of the screen. Reviewing this checklist will provide information about what you need to know before starting the online application process.
- 3). Click on the link to the "Disability Benefit Application," which is located in Step 2 of the four-step process.
- 4). Complete the "Adult Disability Report," which is located in Step 3 of the four-step process. Provide all information regarding your medical and employment history.
- 5). Complete the "Authorization to Disclose Information" form, which is the last step in the four-step process. This allows physicians and treatment specialists to discuss your medical conditions and treatments, if necessary. Sign and mail, or deliver, the form to the Social Security Administration or to the local Social Security field office.
